New patients

If you are a new patient seeking to book an initial appointment with one of our nephrologists, please click below to contact us for a new appointment. Please download and complete the

Confidential Patient Information Form. We appreciate your current referral letter and any

relevant blood tests or imaging to be sent to us via fax or email prior to your appointment

so that your doctor can arrange any pre-consultation tests if necessary.

If you are an existing patient of Eastern Nephrology, and wish to change your existing appointment, please call us to arrange a change of appointment.

Consultation Fees

PLEASE NOTE: Eastern Nephrology is NOT a bulk-billing clinic.

Payment is required on the day of consultation and we claim the Medicare rebate on your behalf if you are eligible and have a valid referral. Please note that if your referral has expired, we are unable to claim the Medicare rebate on your behalf.

Specialist referrals are valid for 3 months,

GP referrals are valid for 12 months.

We do not accept indefinite referrals.

We accept payments in the form of cash, cheque, direct deposit or EFTPOS.

The cost of the consultation depends on the reason(s) of your referral.

SCRIPT REQUESTS

Please note that a script request fee of $30+GST will apply for any scripts that are requested outside your consultations with the doctors.

We encourage all patients to ask for any scripts required during your appointment with our doctors.

CANCELLATIONS

Please note that a cancellation fee of $50+GST will apply for appointments cancelled with less than 48 hours' notice.

Please discuss with us if you cannot make it in for your appointment, your doctor may be able to offer you Telehealth options for your consultation.
